Job Title: Travel Nurse - RN - Case Management
Location: Glendale, CA, USA
Shift: Day Shift (Monday - Friday)
Duration: 13 Weeks
Salary: $40 to $50 per hour

 

Job Summary

We are seeking a compassionate and experienced Travel Nurse - RN specializing in Case Management to join our team in Glendale, CA. This role involves inpatient case management with a strong focus on timely discharge planning. The successful candidate will work day shifts from 8:00 AM to 4:30 PM, with a commitment to every other weekend, totaling a 40-hour work week.

 

Key Responsibilities

• Conduct comprehensive assessments of patients' needs and care requirements.
• Collaborate with interdisciplinary teams to develop and implement effective discharge plans.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Educate patients and families on discharge processes and post-discharge care.
• Maintain accurate documentation of assessments, interventions, and patient progress.
• Adhere to hospital policies and regulatory requirements regarding patient care.

Qualifications

• Experience: Previous inpatient RN case management experience in an acute hospital setting is required.
• Education: Current Registered Nurse (RN) license in the state of California.
• Certification: Case Management certification (CCM or ACM) preferred but not required.
• Skills:
  • Strong critical thinking and problem-solving abilities.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a fast-paced environment.
