We are looking for an experienced, dynamic RN Case Manager to join our team.
Position Summary
As a Case Manager , you will be responsible for the coordination of care with the patient, family, and other members of the team. Our Case Managers work as RN/LPN teams so we need someone with great supervisory and organization skills.
